Liposuction is a surgical technique for removing localised deposits of fat that persist despite a healthy diet and regular exercise. At her practice on Harley Street in London, Ms Anna De Leo, a GMC-registered Consultant Plastic, Reconstructive and Aesthetic Surgeon, approaches liposuction as liposculpture: not simply taking fat away, but reshaping each area so that it flows naturally into the next and suits your frame.
Many people carry fat in places that seem stubbornly resistant to change – the lower abdomen, flanks, outer thighs, inner knees or upper arms – often as a result of genetics and hormones rather than lifestyle. Liposuction can refine these areas through incisions only a few millimetres long. It is not a treatment for obesity or a substitute for weight loss, and it does not tighten significantly loose skin. Used for the right person and the right area, though, it can produce a smoother, more balanced silhouette that lasts as long as your weight remains stable.
01What Is Liposuction?
In liposuction, a dilute solution containing local anaesthetic and adrenaline is first infiltrated into the fatty layer. This tumescent fluid reduces bleeding and bruising and makes the fat easier to remove. Fine, blunt-tipped tubes called cannulas are then introduced through small incisions of around 2–3 mm, and moved in a controlled, fanning pattern to suction fat cells from beneath the skin.
Liposculpture describes the artistry involved: working in different layers, feathering the edges so there is no visible step between treated and untreated areas, and preserving a smooth layer of fat beneath the skin. The goal is a natural contour, not an over-thinned or hollowed look.
Areas commonly treated
- abdomen, waist and flanks (“love handles”);
- hips and outer thighs, inner thighs and knees;
- buttocks and the area beneath the buttock crease;
- back and bra-line rolls;
- upper arms;
- chin, jawline and neck, and in selected cases the cheeks.
Liposuction can also be the first step of body lipofilling, where the harvested fat is purified and transferred to add volume elsewhere, and it is frequently combined with a tummy tuck, brachioplasty or thigh lift to refine the contour.
02Who Is a Good Candidate for Liposuction?
Liposuction works best for people who:
- are at or close to a healthy, stable weight;
- have localised areas of fat that are out of proportion with the rest of the body;
- have good skin elasticity, so the skin can contract over the new contour;
- are in good general health and do not smoke;
- understand that liposuction refines shape rather than reducing overall weight.
If you have significant loose or overhanging skin, removing fat alone may leave the skin looser or more uneven. In that case, a skin-tightening procedure such as an abdominoplasty, thighplasty or arm lift may be recommended, either instead of or alongside liposuction. Lipoedema and other medical fat disorders require specialist assessment and are discussed individually.
Body image is personal, and the decision to change it deserves time. Many women in London tell us they find it easier to discuss areas such as the inner thighs, abdomen or bra line with a female surgeon, and Ms De Leo welcomes questions at every stage. Equally, men seeking contouring of the flanks, abdomen or chest are assessed with the same care and the same emphasis on proportion.
03Your Liposuction Consultation on Harley Street
Your consultation at 64 Harley Street, in the heart of Marylebone, begins with your goals and your history: weight changes, general health, medications, previous surgery and lifestyle. Ms Anna De Leo will then examine each area of concern, assessing the thickness of fat, skin tone, any existing irregularities and the overall proportions of your body, so that treatment of one area complements rather than unbalances another.
You will discuss which areas are worth treating, how much change is realistic, which anaesthetic suits your plan and how to prepare. Liposuction cost factors – the number and size of areas, anaesthetic, operating time, whether an overnight stay is needed and the aftercare programme – are explained clearly so you understand how your plan is built. Many people who search for liposuction near Marylebone or the best liposuction surgeon in London value this whole-body perspective.
In line with GMC guidance, there is a cooling-off period of at least two weeks between your consultation and surgery, and you are always welcome to return with further questions.
04Liposuction Techniques and Anaesthetic Options
Choice of anaesthetic
Depending on the number of areas and your preference, liposuction can be carried out under:
- general anaesthetic, when you are fully asleep – usual for larger or multiple areas;
- spinal or epidural anaesthetic, numbing the lower body while you remain awake;
- local anaesthetic with conscious sedation, suitable for smaller, well-defined areas.
Precise, scar-conscious incisions
Access incisions of 2–3 mm are placed within natural creases, the navel, the bikini line or other discreet sites. They heal as small marks that usually fade considerably, though they may remain faintly visible on close inspection.
Fluid balance and safety
Because fluid is removed along with fat, intravenous fluids are given during and after surgery to maintain balance. Ms De Leo observes safe limits on the volume of fat removed in a single session; when more extensive change is desired, staging the treatment is often safer.
Compression
At the end of the operation, an elasticated compression garment is fitted. It helps control swelling, supports the skin as it settles onto the new contour and reduces bruising. It is usually worn for around four to six weeks.
Good liposculpture is measured not by how much fat is removed, but by how seamlessly the new contour belongs to the rest of you.

05Liposuction Recovery Time
Recovery depends on the number of areas treated, but a typical course is:
- Same day: most patients go home a few hours after surgery from an accredited London hospital; larger procedures may need an overnight stay.
- First few days: soreness similar to deep muscle ache, swelling and bruising, with some leakage of tumescent fluid from the incisions.
- One to two weeks: return to desk-based work for most people.
- Four to six weeks: compression garment worn; gradual return to exercise from around three to four weeks.
- Six to eight weeks: most of the swelling has resolved.
- Three to six months: the final contour becomes apparent as residual swelling settles and the skin adapts.
Lymphatic drainage massage can be particularly helpful after liposuction. The ADLSynergy team, including Tika with her long experience in lymphatic care and scar management, provides support during recovery, along with red light therapy, oxygen therapy and nutrition advice through our longevity programme. Follow-up appointments take place on Harley Street.
06Liposuction Risks and Realistic Results
Liposuction is a common procedure, but it is surgery, and it carries risks:
- contour irregularities, dimpling or asymmetry, sometimes needing revision;
- loose skin if elasticity is limited;
- prolonged swelling, bruising or areas of altered sensation;
- seroma (fluid collection), infection or bleeding;
- changes in skin colour or small scars at the incision sites;
- blood clots, and rarely fat embolism;
- rarely, injury to deeper structures;
- risks related to anaesthesia and fluid balance.
Fat cells removed by liposuction do not regenerate, but the remaining fat cells can still enlarge if you gain weight, so maintaining a stable weight is essential to preserving the result. Before and after photographs can illustrate what liposuction may achieve, but results vary with anatomy, skin quality and healing.
